How to prepare a patient before surgery

Information for patients, parents, or legal guardians before dental sugery under general anesthesia. Please read the information below before signing the consent to a proposed dental surgery under general anesthesia.

Mode of consultation before surgery under general anesthesia.

Before surgery under general anesthesia there is a prior dental consultation during which we define the range of surgery, and anethesia consultation. During the consultation anesthesiologist assesses the health of the child and the degree of risk assosiated with the surgery. During the consutlation patients, parents of legal guardians are obliged to provide accurate and comprehensive answers to questions from the survey. Patients are obliged to make the following medical examination before surgery: ECG, morphology, coagulation tests, urea, creatinine ,potassium, sodium. Patient comes to surgery with the results.

Preparation for surgery

Before surgery in general anesthesia patients can not eat for 6 hours before, and can not drink for 3 hour before surgery. On the day of surgery patients can only drink water, tea (can be with sugar), or apple juice - no later than 3 hours befor surgery. Patients can not drink milk, citrus juice, sparkling drinks (even degassed). Patients can not chew gum. If the patient takes medication it has to be consulted with anesthesiologist earlier. If after consultation with anesthesiologist patient takes medication he/she should only drink small amount of water before surgery. If patient eats something during the 6 hours/or drinks something 3 hours before surgery it has to be necessarily reported to anesthesiologist. In such situation, for the good of the patient, the surgery will be conducted at a later time. Concealing this fact can cause life-threatening complications.

The surgery should be also postponed if the patient has infection with fever, cough, rhinitis, and also in the patien has herpetic changes around the mouth. Final decision is taken by anethesiologist.

Contact lenses, rings, earrings, necklaces, and other personal ornaments should be removed before surgery. Patients should wash of the nail from fingernails. Patient should wear a T-shirt.

Patient who is able to understand should be informed about all consequences connected with the surgery, and should be also mentaly prepared for the surgery. Information should be presented in such way that in does not increase patients’ anxiety. Preferably it should be expained to patient what will happen during the surgery, and all of paients’ questions should be answered before surgery. In a situation that paties had unpleasant experiences connected with dental procedures it is good to expain to patient that during this particular procedure he/she will first fall asleep, andwhen he/she wakes up teeth will be cured. 
After arriving to our health center, in the first stage patient is with his/her parents, legal guardians, or acompanying persons. In order to reduce patients’ fear aneasthetics can give the patient some sedative drugs dissolved in small amount of apple juice. In case that patient can not take the drugs orally it can be given rectally. After taking sedative drugs patient should not walk. After 10-30 minutes from taking sedative patiens becomes sleepy. Then the patiens is transferred to the treatment room. Prior the insertion of intravenous parents or legal guardians go to waiting room.

In order to reduce the pain during intravenouscream EMLA cream should be applied 1.5 hours earlier. In can be applied on the back of right hand, andon the bend of right arm, or on other place indicated by the anethesiologist. You should apply about ⅓ of tube of EMLA (the whole tube has 5g of cream), and tighty stick the foil attached to the tube. You can buy the cream in chemistry on prescription. The same procedure may be applied during blood collection before testing.

Following the recommendations the patient should be vaccinated againts Hepatitis B if he/she was not vacinnated before. In case of first vaccination effective protection is obtained after two vaccinations with monthly break. The surgery can be taken at least two weeks after second vaccination. Despite the fact that in our health center infection with Hepatitis B is minimal ( we have modern sterilization system, and we use disposable equipment) however we recommend vaccination againts Hepatitis B. If the patient does not decide to vaccinate himself againts Hepatitis B, and you sign a consent for surgery you automatically resign of claims arising form infection with the virus.

Please remember to take the health book, hospital information card, and other medical documentation, and give it to anesthesiologist before the surgery.

Treatments and anesthesia.

Before intravenous, parents, or legal guardians will be asked to leave the treatment room. They can stay in our waiting room. 

General anesthesia, also called narcosis involves application of sedatives muscle relaxant and painkillers. Patient will not feel or remember anything from the surgery. Plastic tube enabling breathing, and protecting the respiratory system  will be introduced into trachea. After removing the tube the throat can be sore, what can manifest inself  in hoarseness.
